  • We see individuals across the lifespan from ages 5+.

  • We are currently accepting new patients!

  • At this time, we accept cash-pay only with plans to expand coverage to several of the larger insurance providers.

  • New Patient Evaluation- $250

    Follow Up Appointment- $150

    Plus 1.8% MNCare Tax

  • We offer secure, HIPAA-compliant telehealth appointments, but prefer to limit our patient panel to those who are close enough to travel to our office as well.

  • A referral is not needed. If you are transferring care from another provider who has managed the condition which you are seeking treatment, we will request your previous records.  This is to confirm diagnosis and to learn more about you.

  • At this time, our providers to do not offer these additional services.

  • Yes, our providers provide ESA letters when appropriate.

  • At this time, our providers do not certify for medical cannabis.
